Department of Transportation (DOT) Exam with Preplacement Physical

This exam combines a preplacement physical exam with a DOT certification. Our medical provider will issue a written opinion regarding the prospective employee’s ability to do physically demanding work in addition to granting a DOT card.

• Medical and work history reviewed by an occupational healthcare provider
• Physical exam by an occupational healthcare provider
• Vital signs
• Vision screening
• Hearing test (whisper)
• Urine dip stick
• Documented statement regarding work abilities for job placement
• Completion of DOT form and medical card
