Immigration Exams

 

North Works Airport Clinic provides examinations for individuals wishing to become citizens of the United States.  One part of the immigration process is to ensure immunizations are current according to the Department of Human Services Centers for Disease Control.

The immigration applicant presenting this letter to you has chosen to have their immunizations administered at your clinic at the applicant’s expense.  An applicant does not have to receive a completed series of each vaccine to qualify. The following checked vaccines are the vaccines this immigrant applicant needs to meet the vaccination requirements. 


Vaccinate against the following:


  • DT/Tdap
  • TD
  • Polio (OPV/IPV)
  • Hib
  • Hepatitis B
  • Varicella
  • Pneumococcal
  • MMR (measles, mumps and rubella)
  • Influenza
  • Rotavirus
  • Hepatits A
  • Tetravalent meningococcal
  • Acellular pertussis


 

To complete the medical portion of the immigration exam, written documentation of immunizations given must be forwarded at your earliest convenience to:
